An 85-year-old man who was allowed to drive despite severe heart problems collapsed at the wheel of his car and hit an ambulance, which led to an elderly woman's death, an inquest heard.

Despite suffering mild heart failure, an abnormal heart rhythm, hypertension, arthritis and being heavily medicated, Bryan Dawson was permitted by law to drive.
